Covid Dose 2: bring card noting first dose of vaccine
Covid Booster: bring card noting 1st & 2nd dose
Flu Shot: bring your insurance card – Part time employees or students without insurance are eligible for a free flu shot with short grant form.
All vaccinations are provided walk in only – no appointments.
Under 19: parental consent required (form completion or verbal phone consent allowed)
